Medical Assistance by Telephone:

In all cases, we will need to review your medical record before we can answer any medical inquiries. Only your primary orthopaedic specialist will be consulted, unless the call is considered an emergency. You will be contacted and your question will be answered by one of our physicians, physician extenders or Patient Care Team Members.

Medical Record Information Requests or Form Completion:

Your signature on our Medical Record Request form is required; it gives us authorization to release medical record information, including x-rays. A processing fee will be charged for medical record copies, form completion or x-ray duplication. This process sometimes takes 7-14 business days or longer.

Prescription Refills:

We do not accept requests for medication refills over the phone. Please contact your pharmacy for refill requests.
